Notes Page Overview
The Notes section allows you to create notes for patients, upload and store important documents, as well as find notes easily by using filters. In this section, you will find a concise overview of the Notes section for a patient.
To access the notes page on a patient's profile, you can navigate to the patient profile and select Notes. Additionally, you can access the notes page directly from the patient page as both an admin and a therapist.
You are able to filter by note types to help you find your notes easier using the "Filter By Type" dropdown.
If you would like to print or download a pdf of a patients notes, you will be able to using the correct corresponding icon at the top right of the notes page.
An overview of the amount of notes you can view are shown.
If you would like to upload a document, the "Upload Document" toggle will direct you to a document note.
Select the "Create Note" icon to create new blank notes.
The bookmark icon will allow you keep certain notes at the very top of the page, for easy viewing.
The additional options icon will allow you to view, edit, lock and sign, delete and share notes with patients.
Creating Notes for Patients
This is an overview of how to create notes for a patient in Greenspace.
There are 4 essential component of notes:
Note information - Here you can select the Note Type, Date, Method of note taking and Duration using the appropriate dropdown.
Please note: some of this may look different depending on the note type selected.
Who the note is shared with - If you are a therapist, the note will automatically be shared with clinic admins. Use the checkboxes to indicate who you would like the note to be shared with.
The content of your note can be inputted below - You are able to use many formatting features such as bolding, margins and indents. Just like you would in other word processors.
You can add attachments to your note, such as pdfs - Use the "Add attachment" icon at the bottom left of the note. You will then be prompted to attach your file.
Once you have your note filled out with all the necessary information, don't forget to save.

Please be aware that event notes are shared with patients who have a full account and are overlayed on the patient's results page.

Please note: Note Types and Templates are set by the clinic owner/admins.
Editing Notes
If you need to make modifications to your notes once they are completed, follow the steps below:
Step 1: Navigate to the notes section of your patient's profile.
Step 2: Select the additional options icon for the note you would like to change.
Step 3: Select edit to begin editing the note. Remember to save after making the changes.
Deleting Notes
Step 1: Navigate to the notes section of your patient's profile.
Step 2: Select the additional options icon for the note you would like to delete.
Step 3: A deletion confirmation will appear. Select Delete to remove the note.
Lock and Sign
After creating and saving certain types of notes, providers have the option to lock and sign the note. Once a note is locked and signed, the note is time-stamped with the provider’s electronic signature and can no longer be modified. The following types of notes can be locked and signed:
Session Note
Supervision Note
Treatment Summary
Here are the steps to Lock and Sign a note:
Step 1: Save your note.
Step 2: After saving the note, the Lock and Sign button will appear. Selecting Lock and Sign will mark the bottom of the note with the provider's signature and the date and time that the note was locked and signed.
Once a note has been locked and signed, the note can no longer be edited or modified. You will see that the Edit button is no longer available and the bottom of the note indicates the date and time the note was locked and signed.
While modifications are no longer permitted after a note has been locked and signed, the note can still be deleted by pressing Delete. However, if a note is deleted after being locked and signed, a record of the deletion will be retained and displayed on the patient’s note summary view.
Please Note: To hard delete notes, please reach out to Greenspace Support at [email protected] for assistance. You will be required to provide the Greenspace Patient ID, the date the note was submitted, and the note type. Once a note is hard-deleted, it is not recoverable.
