Patient profile fields are Greenspace's way of collecting patient demographic data. To collect this data Greenspace uses 3 types of forms each with their own customizable profile fields to make sure you gather the data that you want to collect.
Types of Forms
Referral Form
Account Creation Form
Profile Form
The Referral Form is the form that is presented to therapists when referring a patient to another clinic.
The Account Creation Form is the form presented to users who are creating their account for the first time.
The Profile Form is the form presented to patients when they have logged into their account.
Editing Forms and Profile fields
If your organization ever needed to customize a form for the collection of data, you are able to do so in the clinic settings.
To get started:
Step 1: Navigate to Clinic Settings.
Step 2: Navigate to Patient Profiles.
Here you will be able to see the form's sections, fields or questions, question types, questions titles, question answer types and also know which questions are presented in which form.
Please note: Greenspace comes with default forms that can be customized. These are preloaded with default questions that range from general demographic questions, contact information and treatment history.
To edit profile fields, question titles, answer types and more, follow the steps below to get started:
Step 1: Find the section or question you would like to edit.
Step 2: Proceed with Edit Section to enter the edit view for that section.
Once in the edit section view, you may implement any of the changes below. Please refer to both the list, photos and guide below:

Reorder sections within a form.

To reorder a section within a form use the arrows to move the given section up or down within a form.
To edit a "section title" or "section description" select the green pencil icon next to either the section title or description. Proceed to enter your title or description and save.

3. To reorder a question within a section use the green hamburger icon and drag the icon/question to your desired location.
4. To delete a question use the red X next to the hamburger icon. You will be given a confirmation before deleting.
5. To change the question type select the dropdown and choose your desired question type. Please note for single and multiple selection question types, you will be prompted to add your own preset answers. To add or delete answers, select the answers dropdown>edit answers and proceed to add your answers or delete them using the X. Select continue to complete.
6. To change the question title simply select the current title and begin typing your own.
7. To add a hint, simply select "add hint or example" and begin typing.
8. To change which forms you would like your given question to be present in, use the checkboxes to make the question viewable, editable or required on a given form.
9. If you would like to make the answer for a given question searchable on your patient list, check off the "searchable on patient list" box and save.
10. To add a new question you will need to navigate to the last question in the section. There you will be able to use "add question".
Adding a new section
To add a new section to your forms, follow the steps below:
Step 1: Once in the Clinic Settings navigate to Patient Profiles.
Step 2: Select "Create new section".
What happens to the existing Data
Deleting a profile field or response option are the only actions that have effect on your data. If a question is deleted all previous responses will be lost. If a response option is deleted, patients who responded that option will have that section be blank and the responses will be lost.
Reordering, adding a response option and changing a question title have no effect on your data. If response mapping is required please reach out to the Greenspace support team.
