Updates to Existing Assessments
We are also pleased to share that we have made updates to many of our existing assessments. Please take a moment to read through the quality of life changes we have applied to the measures below. These improvements have been automatically applied within the platform—no action is required on your part.
BPI | Question Logic Change
BPI | Question Logic Change
What Changed: Conditional logic has been added to Question 1. If a patient responds “no,” the assessment will now skip the remaining questions, as they are only applicable when chronic pain is reported.
This update streamlines the assessment experience and ensures patients are only presented with relevant questions.
BR-WAI | Updated Symptom Severity Thresholds
BR-WAI | Updated Symptom Severity Thresholds
What Changed: We worked directly with co-author Dr. Brent Mallinckrodt to update the Symptom Severity thresholds in the Results display to align with percentile ranges derived from a large sample (N = 1,786) from the BR-WAI development study. Scores can now be interpreted relative to the following percentile ranges:
Range | Symptom Severity |
16–53 | Lowest 25% |
54–63 | >25% to mean, second quartile |
64–72 | >mean to 75%, third quartile |
73–80 | Top 25%, fourth quartile |
Higher scores indicate a stronger perceived therapeutic alliance. These ranges provide context for individual scores relative to typical client responses, helping clinicians assess the strength of the therapeutic relationship accurately and consistently.
CDC | Updated Item-Level Color Coding and Scoring Guide
CDC | Updated Item-Level Color Coding and Scoring Guide
What Changed: The CDC results display has been updated to standardize item-level color coding and align the scoring legend across items:
0: Not true
1: Somewhat or sometimes true
2: Very true
These updates improve consistency and clarity in how item responses are interpreted.
ChOCI-R-P/C | Updates to Scorer and Symptom Severity Thresholds
ChOCI-R-P/C | Updates to Scorer and Symptom Severity Thresholds
What Changed: The ChOCI-R-P and ChOCI-R-C results displays have been updated to reflect the Total Impairment Score as the primary scoring metric and to include updated symptom severity thresholds plotted as a single data point on the graph.
ChOCI-R-C (Child) Severity Thresholds
Range | Symptom Severity |
0–11 | Sub-clinical |
12–23 | Mild |
24–37 | Moderate |
38–48 | Severe OCD |
ChOCI-R-P (Parent) Severity Thresholds
Range | Symptom Severity |
0–15 | Sub-clinical |
16–27 | Mild |
28–40 | Moderate |
41–48 | Severe OCD |
These updates provide a clearer representation of total impairment and support more consistent interpretation of symptom severity across both parent and child versions.
CHOIS, CHOIS-ES | Revision to Scorer, Subscale Display
CHOIS, CHOIS-ES | Revision to Scorer, Subscale Display
What Changed: The CHOIS and CHOIS-ES results displays have been updated to include revisions to subscales, scoring methodology, graph display, interpretation guide, and severity thresholds.
The scoring approach has been revised to use average scores, ensuring more accurate comparisons across subscales with different numbers of items. This scoring methodology is designed to ensure alignment with the literature and clinical best practices for interpreting CHOIS scores. A Total Symptom score has also been added, incorporating all symptom subscales into a single composite metric.
The updated severity thresholds are as follows:
Range | Symptom Severity |
0.00–0.49 | None / Minimal |
0.50–1.49 | Mild |
1.50–2.49 | Moderate |
2.50–3.49 | Moderately Severe |
3.50–4.00 | Severe |
These updates improve consistency in score interpretation and enhance the clarity of visual and interpretive outputs.
GEQ | Subscales Added

GS-PSS | New Version Available!
GS-PSS | New Version Available!
What Changed: We’re excited to announce that the Greenspace Standard Patient Satisfaction Survey (GS‑PSS) has been updated and is now the Care Experience Survey! Based on your feedback, this update makes capturing care experiences clearer, more meaningful, and more consistent across sites:
Name & framing: From “Patient Satisfaction Survey” → Care Experience Survey, with neutral language that works across clinics, schools, and community agencies.
Updated language for broader contexts: Wording has been refined to be appropriate for a variety of settings, including school boards, FSOs, and other community-based programs.
Stronger alignment with care standards: New items ask whether care meets goals/needs and whether people feel safe receiving care, with space for safety and quality suggestions — supporting organizations in meeting common expectations around involving people in care planning and tracking perceived safety and care culture.
Standard wording: All question text is fixed and non-editable, so experience data is consistent, comparable, and easier to report.
The Care Experience Survey helps you gather richer insights into care experiences while keeping reporting and compliance simple — a modern update from the GS‑PSS that truly reflects the people you serve.
HEADS-ED | Suicidality Flag Added
HEADS-ED | Suicidality Flag Added
What Changed: A suicidality flag now appears in the results display when a patient has endorsed Item 5 (“Suicidality”) in the HEADS-ED assessment.
This update helps clinicians quickly identify patients who may need additional support, enabling timely care and treatment for those who need it.
ICG | Cutoff Value Added
ICG | Cutoff Value Added
What Changed: A clinical cutoff has been added to the ICG results display. A total score greater than 25 is now indicated as exceeding the clinical threshold.
This update helps clinicians more easily identify individuals who may be experiencing clinically significant levels of grief symptoms, supporting timely assessment and intervention.
INQ | 2 Subscales Added
INQ | 2 Subscales Added
What Changed: The INQ results display has been updated to clearly display scores for two subscales, providing more detailed insight into key constructs of interpersonal risk:
Perceived Burdensomeness: Items 1–6
Thwarted Belongingness: Items 7–15
Higher scores indicate stronger endorsement of these constructs. The addition of these subscales supports more nuanced interpretation by distinguishing between different interpersonal dimensions.
RAS-12 | 3 Subscales Added
RAS-12 | 3 Subscales Added
What Changed: The RAS-12 results display has been updated to clearly identify patient scores across three subscales, providing more detailed insight into distinct dimensions of self-appraisal:
Emotion Coping: Items 3, 4, 8, 12
Situation Coping: Items 5, 7, 9, 11
Social Support: Items 1, 2, 6, 10
Higher scores indicate more positive self-appraisals. The addition of these subscales supports more nuanced interpretation by highlighting specific areas of coping and support.

What Changed: A suicidality flag now appears in the Results view when a patient has endorsed any of the following items in the past two months:
I thought of hurting myself
I didn’t believe in my right to live
I told other people that I was going to kill myself
I hurt myself by cutting, burning, strangling, headbanging, etc.
The idea of death had a certain fascination for me
