Patient Merge for Multiple Episodes of Care
Patient Merge is currently part of the Multiple Episodes of Care feature, and only available for customers participating in the beta.
With Multiple Episodes of Care, it will allow you to merge multiple patient profiles into one profile. Episodes of care from all merged profiles will be moved under a single profile, but remain as separate episodes.
How to Merge Profiles
Once you locate the profiles that need to be merged, please follow the steps below:
Before you begin merging, only one active profile can be merged at a time, so make sure the other profiles have been archived. You can use the patient search function to find the profiles by name.
Select two or more profiles you would like to merge and click the “Merge” button.
You will see a popup describing the merge function. Here, you can choose a Primary Profile from the ones you’ve selected. The Primary Profile determines the Display Name, Patient ID, and Patient Record Fields that are retained.
Click “Merge Profiles”, and the merge will occur. Please note, there are a few basic requirements for merge, which may results in error messages if not met. See below for a list of requirements.
Merge Requirements
Type | Description |
One active episode/profile at a time | Patients can only have one active episode at a time. Please end any additional active episodes and archive profiles before merging. |
Same patient type | Only one patient type can be merged (e.g. child or adult). Please only select patients of the same type for merging. |
Single account or delivery email/sms | Selected profiles cannot have different user accounts or delivery emails/number. Please consolidate accounts, ensuring only one user account or contact email/number exists. |
Maximum participant number | The maximum number of participants cannot exceed 4 after merging. Please reduce the number of non-therapist participants to 4 or fewer across selected profiles. |
Same organization | Patients from different organizations cannot be merged. Please select patients that belong to the same organization for merging. |
Integration ID | Patients with different integration IDs cannot be merged. Please select only one patient with an integration ID when merging. |
