Managing Episodes of Care

Overview

The Multiple Episodes of Care feature provides Admins and Clinicians with a simple and efficient way to manage patients’ return to care. A single patient profile now offers visibility into a patient’s complete care journey, with data from both current and past episodes. This not only creates a more streamlined workflow, but also improves data accuracy and access, which is crucial to the successful implementation of MBC.

In the current beta version, the feature will offer basic functionality to create, manage, and edit episodes, with additional enhancements to come.

Creating an Episode

All newly created patient profiles will have the first episode added by default. At the top of each patient profile (under patient name), an episode selector will appear showing the currently selected episode.

To start a new episode for returning patients that were previously archived:

Step 1: Find the archived patient in the patient list

Step 2: Unarchive the patient by removing the “Archived” tag, or clicking “Unarchive Patient” in the dropdown menu for the patient.

Step 3: Complete the 'Start New Episode' form by entering an episode name and start date. Tags from your previous episode will be pre-populated, and you can add or remove any tags for your new episode. Then, proceed to assigning assessments, or start the episode.



Patient Profile Navigation

You will see an episode selector at the top of each patient profile, under the patient’s name. Clicking on the episode name will reveal a dropdown menu showing all of the patient’s current and past episodes. This can be used to navigate between different episodes of care. Content in the ‘Results’, ‘Notes’, ‘Manage’, ‘Profile’, and ‘Summary Report’ tabs will change depending on the episode selected.

Managing Episodes

In the top right corner of each patient profile, you will find a “Manage Episodes” page. Here, you can view a history of past episodes, create a new episode, or manage an existing episode.


You can start a new episode using the “+ New Episode” button


The three dots to the right of each episode opens up a dropdown that lets you edit, end, or delete the episode.


Edit Episode Details” will open a popup with options to edit the episode name, start date, and end date. (In order to edit the end date, the episode must have already ended).


You can also choose to restart the most recent episode. This can only be done if there are no episodes currently active. Previously assigned assessments will not be restored.

Ending an Episode

Ending an episode will assign an end date to the episode and remove all assigned assessments.

There are 3 ways to end an episode:


  1. Starting a new episode

    • If you choose to start a new episode when there is still an ongoing episode, you will be asked to end that episode before starting a new one


  1. Archiving a patient

    • If you choose to archive a patient that currently has an active episode, the episode will end.


  1. Ending the Episode

    • You can also use the dropdown in the “Manage Episodes” page. You will be given the option to immediately start a new episode, or archive the patient.


Patient Profile Fields

With the introduction of ‘Episodes’, patient profile information can be linked to episodes (this can be configured by your organization’s Admin).

Profile sections that are episode-specific have a blue “Episode” label in the top right corner. Each episode will have its own copy of these fields. Any changes made will only affect the selected episode.

When an active or most recent episode is selected, all profile sections will appear under the “Profile” tab.

Patient Experience

Patients with a Greenspace account will also be able to view their results and notes across different episodes. Patients with only one episode will see no changes to their accounts.

If a patient has more than 1 episode, there will be a selector on both the ‘Results’ and ‘Notes’ pages.

  • Current episodes will be shown as “Current Results (Since {startdate})”.

  • Past episodes will appear as “Past Results ({startdate} to {enddate})”

Therapeutic Alliance with Episodes

  • Therapeutic Alliance score will be calculated from all episodes.

  • Prior to Multiple Episodes of Care, when a patient is shared with multiple providers, all providers are impacted by the same TA score, even though they may not be the primary provider at the time.

  • Future updates will allow providers to be assigned to specific episodes, and therapeutic alliance scores will more accurately reflect the episode of care.
