What this feature does

  • Let's users with permission to view and edit assessments in a patient record set an assessment’s completion date to any valid past date.

  • Updates the date everywhere the assessment appears, including the Results tab and Insights.

  • Preserves the original completion date for audit visibility in the assessment calendar view.

How to change an assessment’s date

  1. Open the patient record and go to Manage → Calendar.

  2. Click the completed assessment you want to adjust.

  3. In the modal, choose the new Completion date.

  4. Click Confirm.

  5. Verify the change on the calendar.

  6. Reopen the assessment to see both the updated completion date and the original completion date.

What updates after you change the date

  • Calendar: Shows the assessment on the new date.

  • Results & Insights: Reflect the updated date in views and calculations tied to assessment timing.

  • Audit visibility: The original completion date remains visible alongside the updated date when the assessment completion is clicked in the calendar view.

Common use cases

  • Entering assessments completed offsite or outside regular hours.

  • Correcting delayed data entry.

  • Aligning longitudinal tracking or treatment plans to the actual encounter date.


FAQ

Can I revert the date?

Yes. Repeat the steps and set the desired date.


Will reports reflect the new date?

Yes. Anywhere the platform references the assessment’s completion date, it will use the updated date.

Please note: Currently, users are unable to change the completion date of an assessment to a date prior to the date the assessment was assigned.


Troubleshooting

I don’t see the option to edit the date.

  • Ensure you selected a completed assessment (which will be highlighted in green); this is not available for missed or pending evaluations.


The date picker is blocked or shows an invalid range.

  • Ensure the assessment you select is a completed one. You'll know this if the assessment is highlighted in green with a check mark. This feature is not available for assessments that are pending or were missed.

  • You cannot select a date that already has a completion for the same assessment type. To move it to that date, first change or delete the other completion.

  • The new date must be in the past.

If the issue persists, contact Support with the patient ID, assessment name, and date.
